Add the ToolsEnvelopesAndLabels
or if you want the command to always open at envelope or label
add the ToolsCreateEnvelope
or the ToolsCreateLabel
command from the all commands section of the toolbar customisation depending
on which you actually require.

Okay, let's back up and take this in order:
1. Open the Tools | Customize dialog.
2. Select the Commands tab.
3. In the Categories list, select All Commands.
4. Scroll down to ToolsEnvelopesAndLabels, select it, and drag it to a
toolbar.
Or, as Graham says, you can use ToolsCreateEnvelope or ToolsCreateLabels (or
both). The former has the same envelope icon as ToolsEnvelopesAndLabels; the
latter has no icon, so you would have to add one.
