I did insert a new table and the Field Code would display. Then I
toggled it to display the TOC and everything disappeared.
I saved and closed and re-opened the document.
I played around with the view field settings for a while. Eventually
the table did display but how I got it to display is beyond me.
I feel like I'm in the twilight zone.

At the very least, you should be seeing an error message: "Error! No table
of contents entries found."

Does F9 (update field) make a difference?

Thanks everyone for replying. I F9'ed and nothing happened and I never
saw an Error message. Then I remembered an old work around I was once
shown which was to select the entire document, except for the last para
mark, and paste it into a new document. After doing so, I set up my
TOC and it appeared. They lived happily ever after....for now.

Apparently there was some corruption in the document.
If you want to read more about corrupted documents and templates,
please see: http://word.mvps.org/FAQs/AppErrors/CorruptDoc.htm
