Numbered Outlining

I am using Word 2003

I have never been good at using Word's outlining, but I am determined to
figure it out.  I have been customizing the one of the outline defaults.  The
one that looks like this.    1.  ajdfkdj
                    1.1   ajdldjfljd
                            1.1.1   adjaljdflakdjf
                            1.1.2   ajdajlkajdfklasj  etc.

I have attached all of the outline levels to headings.  But the one problem
I am running into is that it is not increasing the numbers - Above Ex:  1.1.1
is the first line and 1.1.2 should be the seconding line.  I am getting 1.1.1
again on the 2nd, 3rd line etc.  This appears to be happening at all levels.  
Does anyone have any ideas as to what is causing this?  I really need to know
how to outline with numbers correctly.  Please help if you can.

In the Customize Outline Numbered List dialog box, does the
third-level number appear with a grey shading? If not, it has been
typed in, which explains why it won't increment. Make sure to replace
it by selecting the desired format from the "Number style" box. Since,
in the "Number format" box, you cannot tell the difference between a
previous level number and the current one, you may have to clear and
recreate the entire contents of that box.

Thank you for your help - your advice did help me but now I have another but
similar problem.  The outline work fine at first - meaning all levels are
good and numbered correctly then all of a sudden level 3 is not numbering
correctly.  For ex:  1.  x
          1.1   xx
                 1.1.1   xxx
     2    x
           2.1  xx
                  2.1.1  xxx
                           4.1.1.2   xxx

The fourth level does not follow the numbering sequence.  I check the
numbering in Heading 1 for all of the levels and they all seem to be OK.  Do
you have any ideas.  

Second question:  I followed Shauna Kelly's artice on setting up the outline
in the header style.  Per her instructions I gave the outline a name (name
1).  When I went back to make corrections I tried to give it the same name
but was told I had to give it a new name (name2).  How do I tell Word that I
want "name 2 outline"?

If you always return to the Numbering dialog through Heading 1 and always
choose the pane that is already selected, you should always return to the
named list template you created. Make sure that Heading 4 (or whatever style
you have linked to Level 4) is correctly set up, that the first number shown
in the "Number format" box is a shaded 1, and that "Restart numbering after"
is set to Level 3.
