Incorrect Paragraph Numbering

I have created a template with four levels of headings and sub-paragraphs.  
The headings are outline numbered.

If, for example, I have a heading (Level One) such as "4 DEFINITIONS" and I
attempt to create a sub-heading (Level Two) I get
"1.1 Terms"
instead of
"4.1 terms".

This is not solved by right-clicking and selecting "Continue Numbering"

All help appreciated
