When I check the "show chapter numbers" box on the page
number formatting it still does not show the chapter
number. If I say that chapter numbers start with a
different heading type (eg Heading 2 instead of Heading1)
then I get some 'chapter numbers' although they do not
seem to be correct. When I change back to Heading 1 stype
to start the chapter number then I lose the chapter
numbers again in the footers.

So it looks like Word is not seeing your Heading 1 numbers. How are you
numbering your Heading 1 style? Are the Heading 1s in the main body of the
text (not in text boxes)?
