Disappearing StyleRef field in header

Hi,

I have a large document where the StyleRef field on some pages disappears
from the header. I have two different cases:

One where making the Header space larger (vertically extending the bottom of
the header space) reveals the StyleRef field.

One where the StyleRef field is simply gone -- no longer there, not present
no matter how I try to find it.

Any thoughts on how/why this might be happening?

Does this happen with new blank documents too? If so, you might need
to update or reinstall your printer driver.

Are you sure the field is in every header of the document? Note that
you can have up to three different headers (and footers) per section,
namely if you enable "Different odd and even" and "Different first
page" in File | Page Setup, Layout tab. Even if you don't have more
than one type of header, it might be unlinked from the preceding
section, in which case you have to insert its contents separately. See
