Missing Heading Numbers in document Map

When an outline-numbered heading follows a manual page break, the heading's
number does not appear in the Document Map. The heading text is there, but no
number. Why?

I don't know *why* it happens; I'm guessing that it is connected to the
fact that manual page breaks don't necessarily break the paragraph
correctly.

I do know how to work around it, though: Remove the manual page break
and format the paragraph that should start a new page as "Page break
before" (Line and Page Breaks tab of Format | Paragraph).
