Two columns to flow from page to page

I want to create two columns, where I can insert the chapter number in the
left column adjacent to the text in the right column.  Each column needs to
flow from one page to the next, not from column to column.

Is this possible?  I have tried a table instead but it is messy and I
couldn't get a ToC to show correctly within a cell of the table.

If all you need in the left column is the chapter number, try
http://sbarnhill.mvps.org/WordFAQs/MarginalText.htm. Or, if you're wanting a
running chapter number in the left column, use a text box/frame anchored to
the header, using a StyleRef field to pick up the chapter number.
