For some reason, the "default page numbering" on a document has been set to
include the heading number and the page.   I can control this in the footer,
but in the table of contents, it defaults to the default page numbering.  I
am unable to find how to change this in the document - either the format of
page numbers for TOC, or the default page numbering itself.  Please advise.
Thanks.

The TOC will use the page number defined in Format Page Number, regardless
of what's actually printed on the page. You need to clear the check box for
"Include chapter number" in the Format Page Number dialog.
