I created a word document formated to fit onto 5" x 3" recipe cards. In
file/page setup I have defined the paper size as 5" x 3" using the custom
feature. When printing, the recipe card is processed through the printer but
nothing is printed, it comes out blank. MS Word 3 sp3. Epson stylus photo 870
printer.

Does it print if you put A4/US Letter paper in the printer?

Word relies on the printer driver to set the page size and most drivers are
poor with custom page sizes. For individual cards use a standard larger
paper size and make up the difference with margins to ensure that the
printer actually prints on the card.
