Page numbering out of order

I have inherited a document to revamp.  I am running into page numberin
problems

In the footer, pages are skipping numbers, i.e. 1,2,3,4,5,7.

Also, how do I format the TOC area to number pages with Roman Numeral
and the rest of the doc X of Y
