In my headers and footers toolbar, I used to have items on the Autotext
dropdown menu, such as page number and filename. The list is gone. How do I
recreate it?

An empty AutoText menu usually indicates that you have saved a document in
or as Normal.dot (see
http://word.mvps.org/FAQs/AppErrors/BlankDocNotBlank.htm).
