Deleting headers and footers

On page 3 of a 10 page document there is a blank header space that covers almost the whole page.  There is no text in the header and everything that could be in there has been deleted.  I need to get rid of this header.  On all the other pages, the footers contain the page numbers and that's what I want.

Also, on page 4 of the document there is a text box on the left side of the page, approx. 3x4".  I can't get the bottom side of the frame to show.  

Can you help me with these two problems?  I have a deadline for this document as it is part of a book.

Have you turned on Show/Hide to make sure there are no
empty returns on the page?  Also, go to File/Page
Setup/Layout tab and make sure Vertical Alignment is set
to Top for the entire document.
