outline numbering won't work

I have a file plan using the outline view and want to number it using outline
numbering, eg. 1, 1.1, 1.1.1, 2, 2.1, 2.2
but it only give me a straight sequence 1 2 3 4 5 ignoring the different
levels.  How do I get outline numbering to work?
