Can anyone tell me how to create a line in a form in Word?

Word is killing me.  I'm trying to create a form for people to submit
workshop proposals.  I've figured out how to create check boxes, but I can't
figure out how to create a line so people can fill out the form in the word
document.  Any tips?

Word "help" is not helpful.
