phone tags for ad in Word

Hi,

I want to write an ad to sell some stuff using Word. Does anybody know how
to create "tear away phone tags" at the bottom of the page.
My problem is that while the ad text is horizontal, the phone tags should be
vertical.
Is it possible to do this at all?

Publisher has a template specifically for this, but if you must do it in
Word, put a (borderless) table at the bottom of the page, one cell for each
tear-off. You can rotate the text in the table cells.
