Close a Form

I have created a form in Word 2003. The user enters data into the form and
then presses OK and the data entered is written out at various bookmarks on
the Word document.

My problem, and I guess it is fairly basic but I can't for the life of me
find out how to do it, is how do I get the form to automatically close once
the data is written out?

Not to worry. As always, after spending hours trying to find something out,
giving up and asking the experts, I find the answer 10 minutes later.
