How do I create a field to ask the user to enter information e.g. insert name
(displayed on the screen, and allowed to be typed over), which will allow
them to insert text/numbers?

- easiest thing to do is create a new document from the letter & fax
template, then just copy & paste the field(macrobutton) u want. Thats what I
just did, i just wanted the user to enter their name, thats it. hope this
helps

Which I guess is a roundabout way of adding a 'Text Form Field'.

Do this:
View menu > Toolbars > 'Forms'

The Forms toolbar has buttons for adding text form fields, drop-downs, etc.

Add a form field, then right-click and choose Properties to enter a
prompt (default text), select the allowed input etc.

Actually, the fax templates supplied by Word does not use form
fields, but MACROBUTTON fields. See
