Creating a userform that feeds data into a template

Okay, please talk to me like a 3rd grader and explain to me how to the
following project.

I am creating a template to be used throughout the agency. When opened it
will got to a form that will require the user to fill out seven fields. Then
once completing it, the form will populate the template and the user can
simply save as and send down the line.

I've tried to different Help's offers to my question, like How to create a
UserForm, with absolutely no luck...I am using 2007 if that helps.
