When you opened the Tools Customize dialog, which template was showing in
the "Save changes in" box at the bottom. The shortcuts will only work when
that template is loaded.
Similarly, if you make changes in VBA using KeyBindings, you must first set
the CustomizationContext property to the template where you want the
KeyBindings stored.

Thanks for your reply. All shortcuts were assugned to the custom
template. I even tried to start all over again. Plain document and copy
macro's, forms, autorext entries, toolbar, etc. into the new document.
no luch at all.

Do you have Acrobat 7 installed? If so, this article may help.

Hi Jonathan,
No Adobe Acrobat 7 is not installed. However the article was an eye
opener.
Some custom Add-Inns are installed. In de autonew and autoopen macro I
disable these. After disabling I start a macro which defines the
shortcuts.
