Word form to be sent as an attachment

I have word form, which has radiobuttons, checkboxes and a Send button,
which i have managed to create. It needs to be emailed as an
attachment, which the receiver should be able to fill up and email
back.

When the users gets this email, he would open the attachment and fills
up the form and then once he clicks on the send button. It should
automatically attach itself to a new email, populating the senders
address in the To: box.

is this possible?

I am happy if the solution works with outlook as the email client.

Any suggestion would be of great help.
