Adding fields to a document with values based on an input page

Hello,

Sorry if this is in the wrong group, if so please redirect me.

We have a word doc that we use to file information to HQ.  This doc has the
same info listed many times throughout the doc.  IS there a way that we
could create an input page at the beginning of the doc that would populate
the fields within the doc itself?

Example:

We put the employee number in the doc 6 times in different places.  We would
to type it in once and have it populate all of the fields.

Right now we just cut and past or type over the doc from the last time and
save it as another name.
