I am replying to my own message because after more analysis i discovered that
having Tracking turned on even though, changes had been accepted and the
document was saved showing "Final" seems to cause this anomaly.

Please comment if you understand why this happens or if it is one of the
"Known Issues" discussed in the knowlege articles although I couldn't find
any info on this.
