Form Design in Word

I have designed a form and it works well.  However, I have found the need to
allow others the ability to place photos in this form.

When I am in the design mode of my form, what are the steps to allow the
placement of one or many photos in this form?

Mr. Freedman,
I noticed on your website on the zip to city demo that you have an after
hours log with a check box element in it. . . .that actually works like it's
supposed to.
Can you please tell me how that works?
I have been trying to get a form of mine to go on the intranet (yes
intranet) here at work and have the check boxes work like their supposed but
they don't.
On the form, it ask the reader to identify which shift their associate works
by selecting the check box next to either of the 3 shifts listed.
The only way I can get the box to have a check in it is to set the default
to open with a  check.  but it does allow the user to change it.
Can you explain what I might be doing wrong or what step I may have forgot
or  . . .

Jay has given you a solution for your original question, I believe.

I'm assuming you mean a document form (online form) rather than a userform.
If so, you should never be in design mode. This comes up only when you are
using the Controls Toolbox. Instead, use the Forms toolbar for inserting
your form elements. For more about online forms, follow the links at
