VBA Form in Word

Hey All,
I am trying to figure out how to create a form, such as an application, and
when you enter your name in the first field on exit it would populate your
name to the other fields in the document where you would have to type your
name.

Is there any way to do this? I'm sure there is i'm just not sure of how to
do it.

Any help is greatly appreciated.
