Option Button - import into Access

I'm trying to import information from a word template into an Access
database.  I'm having a problem writing code to import the option buttons.  
This document is a survey which has 5 options per statement.  I've grouped
the 5 options by naming them the same name under Group Name. This is working
so only one option can be selected.  I don't know how to retrieve these
options and import the result into the Access Database.

Thanks for any help you can provide.
