Document Open not firing like it used to; reinstall didn't help; urgent

I have had a dot file with a couple modules, one user form, and a bunch of
custom toolbars. Usually word (2000) and this template are invoked from an
Access 2000 database. It's been a fairly smooth running productivity app for a
couple of years. It's all running on a terminal server box; office has been
set up to run with the TS transform properly.

Recently things have started to come apart. The toolbars stopped appearing and
there have been many word crashes that take down Access too. So we uninstalled
Office and reinstalled again using the TS transform. Nothing was fixed, rather
amazingly. (note, I was surprised to see the registry settings retained; is
that normal? Things like default paths for both Access and Word, and all
Tools/Options settings in general. I'd expected the office registry settings
to be cleared out but they apparently were not?)

If the uninstall/reinstall didn't fix, what recourse do I have? Is a reformat
called for?

On other machines with the same software installed, everything is running as
usual. Only this one box (the one that counts) has issues.

As an example of one simple seeming issue, the custom toolbars which are
invoked from the mytemplate.dot document on open never appear. If I open that
dot file either directly or by using the mytemplate.dot as the template for a
doc, the toolbars appear on any other pc with word installed. I can't get them
to appear under any conditions on this box, unless I right click the built in
toolbars, and manually switch them on...they don't remain available the next
time the dot is opened.

I hope someone has a clue on this, it's a big deal if it's stays as broken as
it is now.

I'm not sure if this will help, but I've seen something similar. It seems to
be intermittent, but if I save a template with the toolbars hidden, it keeps
them hidden the next time I open it, but only on the machine I saved it with.
Of course, it may be something more complex but it's worth a shot.
