Medical Form

I am creating a form in Word and would like to have two fields

First one

Provider Type            

Second one

Specialty....

The only problem is that I would like the Second one to filter down to
options that are available by the choice selected in the first one. Basically
if you choose Physician in the first drop down menu I would like it to only
supply the Specialties for the Physicians... meaning it would take out
Hospital or Ambulatory and leave Pediatrics or Internal Med.

Any suggestions??

Greg also has some information on his site on how to do this with dropdown
type formfields as used in a protected document if that is the type of form
that
are using.
