Office Assistant at my company is creating a template. Each time the template
is used the header and footer information will change. Is there some way to
have a box or macro pop up that will prompt the user of the template to
complete this information before opening the template? I was thinking of
something similar to what happens when you use a letter template.

Thank you very much for your assistance.

Hi Carol

As a start, see
http://www.word.mvps.org/FAQs/Userforms/CreateAUserForm.htm

If you need more detailed information once you get going, post back.

Hope this helps.
