I have created a questionnaire in word & have added a button in the file so
that when it is pressed it attaches the file in the sender’s e mail.

To do this I have used    " ActiveDocument.SendMail "

This is great but wish to automatically add the e mail address & if possible
send the e mail automatically!!

Any help would be appreciated.

You will have to write an extensive macro that does this. if you are using
Oulook as your mailing client then below link should help
