Needing to change document generator

Hello All

Currently, we have a document at work that we use to generate a form for
customers to fill out.  Its really simple, it takes their social and a date
assigned by us, and converts them to a barcode at the bottom of the document.
I am wondering how we can alter the document to also have a form field for
adding their name or what have you.

The person in charge of this document is no longer in our office.
