Cross reference a text form field?

I have a document and I'm trying to figure out how it was created.  I'm not
sure it's cross referencing a form field or not.  When I open the document
there are 2 form fields.  When text is added to the fields it auto-populates
fields in the header.  When I toggle the field codes in the header the code
is either REF PON \h  \* MERGEFORMAT* or REF LotNo \h  \* MERGEFORMAT*.  
Can someone explain how this works.

Its cross referencing bookmarks "PON" and "LotNo " which may or may
not contain formfields.

I must have been blind yesterday.  When I looked (or thought I looked) for
the bookmarks I didn't see any.  I am so sorry for wasting your time.
