Hello,
I have a word 2003 template with several text fields, then two check boxes
that are "either or choices" and then numbered 1-9 items, each with a check
box on the left and a text box to the right for comments.  Any or all of the
1-9 check boxes can be selected at the same time.  I am trying to create a
Userform that will show up on opening the template so it could be filled in
and the data inserted in the Word template.  I have the first couple of text
boxes working with bookmarks, but need help with the check boxes.  The
template was created by someone else but I can change it.  I am new to
Userforms, but I work in Training and IT and am easily trainable.  Would
appreciate any help.  Thank you.
