mark checkbox(es) on opening a word document

Hello,

Is it possibible when I open a template with a range of 5 checkboxes, that a
dialog appears and let me choose one of the 5 checkboxes. Once chosen the
selected checkbox is checked.

Why do you want a dialog? Why not just click the checkbox?
