Simple web form using word?

I wish to produce a very simple web form for a questionaire.  I note
how to produce a form in word with simple text fields, drop downs etc.

There is a web form version and have been told if I produce one then
it is simple for someone else to place it on the server etc and send
me a text file by email with the results.

However, the web form does not seem as straightforward.  Is there a
site where I can find a dummies guide, basic instructions to doing
radio buttons etc.

Along with tutorial there are perhaps a dozen examples provided on this page
