If you setup a form with a submit button what do you need
to put in the action field of properties?  Where is the
info saved to etc.

Here's one example of a form.

Unless you have MYSQL, PHP, ASP or a server-side method to write the data to
a text file? the rule of thumb is email to yourself. NOTE: This can get
complicated because the including of your email address leaves it open for
spam.
