Haven't a clue in Word.

Hi Grapeape,

I'm assuming you're asking about doing this on a Word
created web page rather than in a regular Word document?
You didn't mention the version of Word you're using but
see if looking in Help for 'Create a Web form' applies in
your version.  The forms can be created using the 'Web Tools'
toolbar.
