I have designed a survey form in Microsoft Word whereby
the recipient can fill out the form, click on the "submit"
button and the survey form will come back to me via
email.  In testing I get a notification for email, but
nothing appears.  What I would like is the survey form to
show up filled out by the recipient.  That way I can print
them up for compilation.

I was told the problem can be in the "encoding"
information, but I am not sure.

Any help would be greatly appreciated.

Thank you for your reply.

Therin lies my dilemma, I think.  I used Visual Basic to
design the survey form.  Under properties for the "submit"
button would the URL go
under "Action", "HTMLName", "encloding", (Name)?  I am not
sure.  The help section say the email address needs to go
under "Action" which I have done. In addition, how would I
find the URL.  The form is saved to my Desktop as a
webpage?

Any ideas?

Mike,
         Here's a link to a definition of URL:

Bob's request for the URL of your web page which would be located on the
open internet is the imperative way for assisting you.
If the web page is located on your desktop and not the open internet:
EX: http://www.microsoft.com
than there is no way Bob can view the source html and provide a solution to
your problem.

Neither will a web page with a form function via email.
Forms require an internet presence to function as designed.

There may be a way to do so in VBA, however somebody with VBA knowledge will
need to advise you.
