I've checked third party add-ins but that does not seem to be the problem.

When I am in Outline display, and the section is collapsed, in addition to
the plus sign there is a dotted line under the text, how do I remove that?
The plus sign is enough to tell me that the section is collapsed, I don't
need that dotted line, it's quite distracting.
