Hi. Every time I open my Word 2000 calendar a Control Toolbox toolbar is
floating near the top. I've gone to Customize and unchecked it numerous
times but it returns when I open the calendar. It doesn't happen with any
other file I open.

I need to create a form that multiple users can fill in and add pictures to
and email to me.  I would like the form to prompt them for the info. I
ultimately want to save this data to a database.  I am using Office 2003.
