I have often imported contacts from a single EXCEL spreadsheet into Outlook
2003 Contacts.  I just tried to do so again but the import process did not
work correctly.  I then noticed that the EXCEL file contains four separate
sheets.

Contacts in messages to be forwarded do not appear as names but as tildes and
question marks. Can that be changed?

Maybe this isn't the optimal way to do what I want, but there is not an
option in the contact window under Actions to create a new note for the
contacts.  You can New Letter, New Appointment, New Task, New Message, etc.
