Is outlook forms the easiest way to communicate through forms?

i have numerous excel spreadhseets which i want to allow
my users to send to me in an email format which looks the
same as the spreadsheet. Because what i want to do is.

The setup - WinNT server, Exchange 5.5 and Outlook 98 Clients.
I created a public folder ("bug reports" based on Task items) and a custom
form (based on the Task again).  One client set up a new computer (XP Pro)
