Mastering Outlook 2000 and it refers to three classes of forms: personal,
organizational and folder.  Then it tells me how to open a form for
customization.  Tools | Forms | Choose Form.  Ok, that works...  Then ...

publish my form to my Personal Forms Library I get the following error:
