I have created an outlook form with text boxes, labels, check boxes etc...I copied the exact form from the compose page to the edit read page however when I send the form only the values from the "Notes" section is displayed to the recipient? any suggestions?

I was trying to customize the Phone log form in Outlook w/BCM and now all I get is a blank grey form, with no input fields. Is there a way to reload the original form. Like a moron I saved the new form under the old name. HELP IS GREATLY APPRECIATED.
