How do you delete a form in the Personal Forms Library?

How can I have a form in an email that people fill out and then reply back to
me with their answers.  I would like to have checkboxes and boxes for them to
fill out.  General questions with checkboxes like how do you like to be
contacted:

After using the HideFormPage method to hide a ModifiedPagesForm, if we Show
this form again using ShowForm, controls like ComboBoxes does not have any
more possible values.  Either we populate this Combobox from ADO with get
rows or filling them with addItem.

Is there a way to take existing outlook contacts and import them to a custom
contact form I have created?

is I created my company letter head in Outlook with Word as the editor
with the logo inserted.  I did a select all and copy.  I then open up a

I'm trying to setup a rule where when a message is sent, then it
automatically gets copied into a specific folder (for IMAP purposes so that
the sent file gets uploaded to the server).  I've found multiple websites

I want to create an email in my website that is already set up. When
customers wish to order they click a button the email appears and in the main
body of the email customers fill out the order information and send it to me.
Is this possible to do?  If so how do I do it?

I created a custom Outlook contact form and I can view the form when I select
Action --> New (the name of the form). Then, I select the the contact form
and all of the customized fields a visible, however when I do an import (with
a csv file -same fields) the default contact form ...

We have a custom form published to the Organizational Forms folder and set
NOT to send the form definition.
The form works fine for most of us but we have found some users that the
code will not execute for.

Also cant forward email with forms.
