I just upgraded from Outlook XP (2002).  All my old forms
appear blank. I went through the usual routine of
clearing my cache, and reloading each form.  I see the
form, but none of the custom actions work anymore.  

I created an .oft form for the subject form.  The "message" area of the form
was created using the Field Chooser and I used the "message" field.
When I try to share this form with others and old signature of mine shows up
within the message.  

I need to add one field to the Task form.  I really don't want to create a
second page for just one field.  It is the "Requested By" field.  Since I
will be saving this as a custom form is there anyway I can add fields to the
first page (Task)?
