This is my first try at a basic form and I'm stumped.
Using Outlook 2003 SP2/Win XP Pro SP2.  No Exchange server, just an ISP for

I created a custom contact form in Outlook 2003 SP2.  I saved and published
the form and am able to use it. However any information I input into the
second or third pages does not save.  Only the information on the first page.
