I have Outlook 2003.  I created a form based off a Message.  This form as the

Is there a way to setup a shortcut to the personal forms library? I would
hate for my users to have to go:
Tools->Forms->Open Form-> Select the library and the form.
I wrote a macro and made a button for it, but a shortcut might be easier.

I have made a simple form with two response forms. The forms are deployed to
the user's personal form library. When they attempt to use one of the reply
forms they get the following error:
You do not have permission to send to this recipient

Is there anyway possible to add an attachment to a form?  Can the information
on a form  be pulled directly from the form and added directly into a
database?  The form will have two combo boxes, a text box, and an attachment
