I have created a form based on contact item. I am able to create contacts using this form. When I attempt to export to excel, none of the new fields I created are showing up to be exported. How do I get the export to use the new form and not the default contact form.

We have a user who has Outlook 2007, when she opens the email sent from a 2003 user, the form is blank. She has been able to publish the .oft ok and when she sends the form to a 2003 user, we can open and read it ok.
