I have a contact in address book. When I send him an e-mail, in my Sent

I have a contact list in a program called first choice, and I need to
convert it into outlook 2007. When I try to import from excell I don't have
similar fields. So I am trying to make a new form. How can I replace the form
for a new contact with one that I've created?
